Annual Blood Screening Program
The Board of Heath is offering the Annual Blood Screening Program at the Senior Center on Tuesday, September 23, 2008 ( cost is $20.00 cash – no checks accepted). It will consist of a CBC, Chem 23, and Lipid Profile, with results sent to your private medical doctor. The cost of a T4 (thyroid) blood test is $24.00 cash. Homocystein level, an amino acid that, when elevated, may lead to blockage in the arteries, is an additional $30.00 (cash). PSA testing, a blood screening tool for detection of prostate cancer, will be an option for all male participants as a cost of $30.00 (cash).
No blood test additions or substitutions. This program is OPEN TO ALL ADULTS, 18 YEARS AND OVER. Appointments must be made in advance of the test date at the MUNICIPAL BUILDING.
This is an excellent opportunity to check on your body’s internal wellness.
*NOTE: Sign ups will be held at the Municipal Building – 85 Humboldt Street .

TESTING DATE: September 23, 2008: 7:30A.M. – 9:30A.M.

At the time of registration, please bring: 2 stamped envelopes – your name and address on one and your physician’s name and address on the other.
*NO ONE WILL BE TESTED UNLESS THEY FURNISH THE NAME AND ADDRESS OF THEIR MEDICAL DOCTOR.
Minors may be tested with parental consent.
A twelve (12) hour fast prior to your appointment time is required. Refreshments will be served at the Senior Center.
You must register and pay in advance – no changes in the type of test.
